Black Medicare and Social Security Commissioner HA-17-6-13

 

A bill to amend the DI tax rate to 2.61%, 1.305% for employees and 1.305% for employers under Sec. 201(b)(1)(S) of the Social Security Act 42USC(7)II§401 and the OASI tax rate to 9.79%, 4.895% for employee under 26USC(C)(21)(A)§3101 (a) and 4.895% for employers under 26USC(C)(21)(A)§3111 (a) without increasing the overall 12.4% OASDI or 15.3% OASDI and Hospital Insurance (HI) tax-rate under 26USC(A)(2)§1401.  To require the Chief Actuary to account for SSI in the 2014 Annual Report of the Board of Trustees of the Federal Old-Age Survivors Insurance and Federal Disability Insurance Trust Funds. To require the Chief Actuary to account for Medicaid in the 2014 Annual Report of the Hospital Insurance and Supplemental Medical Insurance Trust Funds.  To commission an empirical survey of Social Security beneficiaries by race at the request of  the President, to investigate the possibility of statistically significant discrimination against blacks in the administration of social security benefits, aiming to redress this racial disparity with SSI on the rational basis of poverty under Title VI of the Civil Rights Act of 1964 42USC(21)V§2000d-1.  To eliminate discrimination (disclaimer) against naturalization papers in the administration of social security cards, state ID and passports. To discuss paying for SSI with OASDI without raising tax rates until 2020 to reduce the deficit.  To purchase the rights to the balanced federal budget for $1,000 and Hospitals & Asylums work in general from the author for $1,000 a month DI.  Be it enacted by the acting Commissioner of Social Security Carolyn W. Colvin. The proceeds of the tax provisions of the ACA, which extends Medicare taxation to income, without limit, are not evident because they were stolen by the General Fund, but there is no cause for concern for the solvency of Medicare in the immediate future only interference with health care matters by the federal government.  The names of the CMS administrator Ms. Tavenner and acting Chief Actuary Mr. Spitalnic warn against dependency upon health care providers who can no longer be relied upon for moral support, as of the annual report dated May 31, 2013 (nerds beware)!!!

 

CHANGE 19th Draft HA-31-5-13

Cardiology HA-20-4-13                                                                                                                                                  

 

The heart is a four-chambered muscular structure.  It is about the size of a fist but can get much larger with disease.  An adult weighing 160 pounds has about 5 quarts (4.7 liters) of blood in their circulatory system.  The heart beats at a rate of 60 to 100 times per minute.  Of the estimated 50 million, 166 out of 1,000, with unhealthy levels of lipoprotein in their blood, 7 million Americans feel angina (23 in 1,000), 1.5 million will suffer an acute myocardial infarction (heart attack) for which 550,000 will be hospitalized (1.8 out of 1,000) and of the 2.4 million people who died in 2004, 666,000 died from heart disease (2.2 out of 1,000) and 150,000 from stroke (0.5 out of 1,000).  In 1998 these death rates per 100,000 people from heart disease in the United States were 211.8 for black non-Hispanics, compared to 145.3 for white non-Hispanics, 101.5 for Hispanics, 106 for American Indians and 78 for Asians.  Coronary atherosclerosis with greater than 75% stenosis, involving more than one of the three major vessels is present in 80 to 90% of victims; only 10 to 20% of cases are of nonatherosclerotic origin.  Atherosclerosis occurs because a roughening of the endothelium of the arteries by toxins known as cardiac glycosides causes hyperlipidemia when fats (lipids) stick to the wall forming a plaque which can block (occlude) an artery causing ischemia of the cardiac muscle that dies as a result of lack of oxygen in a heart attack.  Angina pectoris, chest pain, can be passing or heart attack damage can take five days to seven weeks to heal barring infection, toxic exposure or animal products. Aspirin, (acetylsalicylic acid (ASA) is known by hospitals to reduce ischemic stroke risk by about 25-30 percent in three to four years but anticoagulants are highly associated with certain heart surgeries.  Statin blood cholesterol lowering drugs, reduce heart attacks by 40 percent and strokes by 30 percent, in six years, or better, and should be prescribed to heart attack patients on discharge, with a vegan diet and exercise program, but cost $2 a pill.  The FDA required warning for Digitalis and related cardiotonic drugs for human use in oral dosage forms, states "Digitalis alone or with other drugs has been used in the treatment of obesity. This use of digoxin or other digitalis glycosides is unwarranted. Moreover, since they may cause potentially fatal arrhythmias or other adverse effects, the use of these drugs in the treatment of obesity is dangerous" 21CFR§201.317 to which could be appended, "Hawthorne is the supreme herb for the heart". With Digitalis as the competition, the ABC of adrenergenic blockers developed since the 1970s have been abused in the treatment of arrhythmias and congestive heart failure and are the second leading cause of fatal drug overdose.  Rauvolfia serpentine alkaloids or hydrochlorothiazide (HCTZ) that cured malignant hypertension in the 1950s or felodipine may be preferred for hypertension for which medicine is essential.  Hawthorne (Crataegus laevigata) is the only medicine that accurately treats the entire constellation of congestive heart failure symptoms including high or low blood pressure and arrhythmias without any known side-effects and clinical trials are needed to make it law or prescribe it at the hospital.  Group A Streptococcus pyogenes that causes sore throat and fever in children causes 50% of rheumatic heart disease and is best treated with penicillin but most antibiotics work. Doxycycline, not for use in children under 9, is the only antibiotic that treats Staphylococcus aureus.  Kidney neglect, malabsorption, resistant streptococci and adverse reactions caused by Bactrim, the only drug indicated for the treatment of E. coli, the most frequent cause of kidney infection, could be avoided if the metronidazole (Flagyl ER) monograph for GI, bone and joint infections of bacterial or protozoal origin contained an indication for Escherichia coli and probiotics were used as an adjunct and for two weeks after antibiotic use. Cocksaxie B and Echo virus can cause a serious myocarditis that is best treated with Human Immune Globulin IV and patients who may be discriminated against because corticosteroids are the best home remedy, are forbidden from participating in organized athletics. Candida albicans often infects the GI and kidneys of immunocompromised patients and is best treated with the broad spectrum antifungal Sporanox (itraconazole) that is strongly contraindicated in congestive heart failure due to adverse reactions with Digitalis and antihypertensive medicines. Cancer is an idiopathic disorder now that the antibiotic, antiviral and antifungal properties of antineoplastic drugs were removed from the drug monographs as was the prescription of Sporanox (itraconazole) as the primary prophylaxis for myeloma, leukemia, bone disease and graft versus host disease (or other neoplasm) caused by fungal aspergillosis, candidiasis, histoplasmosis or mucormycosis, that are hypothesized to cause most cancers and intoxicate the human to kill human cells.  Rational use of $1 athlete's foot crème (clotrimazole) and not the antifungal foot powder spray (toftate) that causes diffuse pain and angina, is often effective.  A vegan diet and daily minimum of 50-100 push-ups, 50-100 crunches, 3 mile run and stretches are prescribed by the military for sedentary officers to be free of angina and old age.

To transfer Chapter One Navy Hospitals, Naval Home, Army and other Naval Hospital, and Hospital Relief for Seamen and Others §1-40 to Chapter 10 Armed Forces Retirement Home §400-435 and write a new Book.  This Chapter shall change the name of the Department of Defense (DoD) to the Military Department (MD).  The US Military employs an estimated 2.8 million US citizens including 600,000 civilian employees. Since its foundation the US military has suffered nearly 1.3 million casualties in 13 wars.  There are reported to be 26.4 million US veterans.  The U.S. is on track to reduce their arsenal from 10,000 warheads, to no more than 1,700 to 2,200 nuclear warheads by 2012.  Eliminating various Cold War weapons systems can save $50 billion from maintenance, redeployment from Iraq and Afghanistan can save another $50 billion.  Surplus military bases and assets shall be sold and surplus funds returned to the Treasury.  Military spending must be limited to $500 billion annually.  Military commanders must prohibit the use of force, environmental modification and biological experimentation.  Compensation must be paid by the State for casualty, injury and property damage to civilians caused by military actions.  For lower cost humanitarian missions the US shall veto Chapter VII of the UN Charter and promote humanitarian missions that pay payroll and corporate taxes, less social insurance and deductibles, to the general treasury of any occupied developing nation.  Democratic peace theory holds that liberal democracies tend to be more peaceful than authoritarian or totalitarian states.  Civilians are customarily compensated for injury, casualty and damage caused by state military action; Quiz…56

 

Book 3 Health and Welfare (HAW)

 

To amend Chapter 3 National Home for Disabled Volunteer Soldiers §71-150, to reduce the deficit to 2.6% of GDP in FY 2012 and 2.5% of GDP in FY 2013 under Sec. 148, to amend the Disability Insurance (DI) tax rate to 2.61%, and the Old Age Survivor Insurance (OASI) tax rate to 9.79% without increasing the overall 12.4% OASDI or 15.3% OASDI and Hospital Insurance (HI) tax-rate, to fix a 3% Cost-of-Living Adjustment (COLA) for social security benefits, to limit medical cost increases to less than 3%, to improve accounting, to limit the term of the Social Security Commissioner to two years, to replace Administrative Law Judges (ALJs) and representative with licensed social workers and non-social worker representatives, to authorize SSA to settle legal and medical malpractice, to finance halfway houses with SSI, to provide Medicaid for free to those earning less than 150% of the poverty line and for reasonably priced premiums for everyone else, to prohibit medical billing, to nationalize health insurance assets, to ratify ILO Conventions 132, 156 and 183, to afford a 1% FICA tax for international development, to pass a Balanced Budget Amendment, and to eliminate the income cap on Social Security contributions to guarantee all Americans an income of $1,000 a month.  During 2011, an estimated 148 million people paid FICA social security payroll taxes.  One-in-six Americans, 55.4 million receives a Social Security benefit.  In 2011, 44.8 million people received OASI benefits, 15 million received DI and/or SSI benefits, and 48.7 million were covered under Medicare.  In 2010 DI paid 9.9 million beneficiaries and SSI paid 7.9 million beneficiaries there is considerable overlap.  Total benefits paid in 2011, including SSI, were $785 billion. Total income including $49 billion from the General Fund to pay for SSI was $830 billion.  Assets grew to $2.7 trillion. In 2011, Medicare covered 48.7 million people. Total expenditures were $549.1 billion. Total income was $530.0 billion. In 2008 Medicaid served more than 52 million Medicaid beneficiaries costing more than $305 billion.  In 2011 38 million people used food stamps at a cost of $53.6 billion.  In 2000 125 million people were covered by unemployment insurance and in 2010 there were around 42.4 million beneficiaries.  TANF beneficiaries have decreased from 14.2 million in 1993 to less than 5 million in 2003.  In 2001, 6.9 million custodial parents were due an average of $5,000, $34.9 billion due and $31.9 billion (62.6%) received, averaging $3,200 per custodial family and another $900 million was paid voluntarily. Book 9 Public Health Department (PHD)

 

To amend Chapter 9 Hospitalization of Mentally Ill Nationals Returned from Foreign Countries §321-329 that was transferred to Chapter 4.  Common infectious diseases accounted for 40% of all deaths in 1900 but they accounted for only 4% of all deaths in 2000. Cardiovascular disease (CVD; heart disease and stroke) accounted for 14% of all deaths in 1900 and for 37% in 2000. Cancer accounted for only 4% of all deaths in 1900 but for 23% in 2000.  In 1900, infant mortality was 162 per 1,000 live births and life expectancy at birth was only 47 years. In 1940, infant mortality was 63 per 1,000 live births and life expectancy was 55 years. In 2000, infant mortality was 7 per 1,000 and life expectancy was 77 years.  People suffering from chronic illnesses must throw away their war-robes and death beds, eat a vegan diet of fruits, vegetables and whole grains and exercise regularly.  Medical residency rotations must be limited to less than 60 hours per week.  Payments to highly paid specialists and surgeons must be reduced and primary care and essential medicines promoted.  To secure people against the dangers of the Public Health Service (PHS) and at long last, graduate from the Department of Health Education and Welfare (HEW), with a Public Health Department (PHD), the Centers for Medicare, Medicaid and SCHIP (CMS) shall change their name to Medicare and be transferred with all other Mandatory Benefit programs to the supervision of the Social Security Administration (SSA). Children’s Health insurance shall be financed with 100% of the proceeds of the Attorney General’s Master Tobacco Settlement.  The Substance Abuse Mental Health Services Administration (SAMHSA) shall change its name to Social Work Administration (SWA) and in time become an independent social work agency also responsible for the Agency on Aging and Children and Family Services.  The Drug Enforcement Administration (DEA) shall change its name to the Drug Evaluation Agency (DEA) and be transferred to the Food and Drug Administration (FDA).  Whereupon the DHHS may adopt the name Public Health Department (PHD) on the condition that an Education Division (ED) to the Agency for Toxic Substances and Disease Registry (ATSDR) be created to secure toxic laboratory supplies.  Epidemiologic statistics shall be improved.  The abuse of bio-medical laboratory supplies is prohibited, infectious disease quarantined and trans-fats banned, to improve public health and hygiene; Quiz…1283
